Case Manager:

Annual Pay Range: $51,451.97 - $64,832.46

Hourly Pay Range: $26.385626 - $33.247415

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS

Clackamas County's Behavioral Health Division (CCBHD) is seeking an energetic and adaptable case manager to support the Involuntary Civil Commitment Program and Psychiatric Security Review Board Program. This position will require the individual to be familiar with Civil Commitment and Psychiatric Security Review Board laws, rules and regulations and to be willing to provide monitoring services for individuals who have been civilly committed or who have been placed under the Psychiatric Security Review Board after being found Guilty Except for Insanity of felony crimes. It requires the ability to communicate and work well with a variety of community partners including hospitals, law enforcement, the circuit court, insurance and mental health providers.

Required Minimum Qualifications/ Transferrable Skills:*

• Qualified Mental Health Associate (QMHA) or (QMHA- R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) at the time of hire

A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:

(a) Bachelor's degree in a behavioral sciences field; or

(b) A combination of at least three years relevant work, education, training or experience.

Additionally, QMHAs must demonstrate the ability to communicate effectively, understand mental health assessment, treatment and service terminology and apply each of these concepts, implement skills development strategies, and identify, implement and coordinate the services and supports identified in an ISSP.

• At least 1 year experience providing case management support to individuals who are in mental health crisis

• Experience, knowledge and commitment to serving a diverse population and promoting equity, diversity and inclusion

• Experience working collaboratively with community partners, collateral service providers, law enforcement agencies and co-workers

• Experience in assessing client needs and supporting treatment plans

TYPICAL TASKS

• Performs case coordination and monitoring services for the Involuntary Commitment and Psychiatric Security Review Board programs, including coordinating pre-commitment investigations and communicating with hospitals, insurance providers, CMHPs and the Circuit Court; and performing commitment, Psychiatric Security Review Board and trial visit monitoring functions per ORS 426 and 161 and OARs 309 and 859. Examples of duties would be approving and/or monitoring conditions of trial visit and conditional release; coordinating services with providers and family members; face-to-face meetings with clients who are civilly or criminally committed; making reports to the Court or the PSRB; and, when necessary, arranging for revocation of community status.

• Provides care coordination services to adult clients of mental health crisis program and callers of the mental health crisis line. This includes developing and coordinating resources to provide consumers with referrals and connections to appropriate resources and providers to meet identified treatment objectives; monitoring client progress and ensuring treatment plans are carried out; visiting clients in homes, hospitals and residential care facilities; noting changes in mental/physical condition and deficiency in care being provided; reporting and gathering information concerning abuse and neglect complaints; assisting in discharge planning.

• Conducts interviews with patients, relatives, and other individuals, as well as other agency treatment personnel and collects critical information necessary to complete comprehensive needs assessments that include psychosocial, financial and physical factors; evaluates functional impairments of consumers; develops and implements treatment plans; determines appropriate treatment, training and care service needs. Refers clients to other internal providers as assigned.

• Writes case plan summaries, reports, and letters and maintains electronic and paper case records in accordance with OARs and Agency procedures; develops, submits and modifies necessary documentation to implement payments and program benefits as necessary; completes program related assignments as required.

• Provides counseling, training and support to clients, families and providers in regard to placement and treatment plan choices, personal goals, daily living skills, financial planning, vocational and job readiness and medication management.

• Coordinates with internal and external health care providers and social service personnel to obtain services for consumers or to participate in problem solving; consult with other staff as assigned; meet with community agencies as assigned to coordinate care and meet client needs.

• Advocates for clients with family, providers, public and private agencies, law enforcement agencies, justice system, schools and others; responds to emergencies and negotiates resolution of problems.

REQUIRED KNOWLEDGE AND SKILLS

Knowledge of: Methods and techniques of case management and care coordination and social interaction and development related to mental health clients in mental health crisis; rules, laws, regulations, and ethics governing the treatment of mentally ill consumers and their families, particularly those involved in the civil commitment process or under Psychiatric Security Review Board jurisdiction; principles and techniques of interviewing, counseling and crisis intervention; procedures used in gathering, evaluating and transmitting information in both written and verbal form; basic math; English grammar and composition; record keeping procedures and techniques; interpersonal and public relations principles and techniques; regulations and ethics governing the issues of confidentiality; regulations, policies, services and mission of specific program assignments; office procedures, practices and equipment including personal computers and related software; community resources for clients; problems and issues of disadvantaged persons; principles of cultural competency; agency services and criteria for eligibility; mental health, legal, medical and social resources in the community; applicable OARs governing service provisions to adult consumer; standard clinical documentation procedures and requirements; evaluating appearance and conditions of clients, homes and care facilities; and public social service program requirements; laws, rules and regulations applicable to assigned program.

Skills and ability to: Develop and implement consumer treatment care plans and monitor consumer progress toward established treatment goals; prepare and present written and oral presentations; work effectively with other mental health professionals and as a member of a professional mental health team; follow agency specific record keeping procedures, including ability to work within an electronic clinical documentation system; communicate effectively, both orally and in writing. Prepare and maintain accurate and legible records; interpret policies, procedures and requirements applicable to the program; take appropriate action as a representative of the County to ensure public safety while respecting individual client right; effectively handle multiple time sensitive tasks in an efficient and organized manner; and monitor and motivate individuals in mandated treatment settings.

WORK SCHEDULE

This position works 40 hours during a standard workweek of Monday through Friday. Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. The Clackamas County Behavioral Health Division strives to provide excellent mental health and addictions services to Clackamas County residents who receive Medicaid and Medicare, or who may be uninsured. We offer services through partner providers located throughout the county. We also offer direct service through our Crisis Services programs.
