FORT BEND COUNTY DEPARTMENT: BEHAVIORAL HEALTH SERVICES

JOB TITLE: CLINICAL CARE COORDINATOR

 

JOB SUMMARY:

Focuses on the development of specialized interventions and services delivery system to at risk youth with mental illness and their families. Participates in planning of programs, policies or objectives for own work group and department.

 

DUTIES & RESPONSIBILITIES:

Conducts initial and ongoing assessments to youth while enrolled in the Juvenile Diversion Program. Develops comprehensive service plans that meet the participant and family’s needs, quality management standards, and 1115 Waiver parameters. Makes all required and needed contacts with participant, family, caregivers, case planner, medical consenter, juvenile probation, courts, service providers and other collaterals. Documents services timely and accordingly. Responsible for data collection and reporting to meet requirements of the 1115 Waiver Project and Behavioral Health Services Department. Ensures implementation of best practices for target population. Assists with report writing as needed for project documentation. Works collaboratively with other agencies and families. Develops resources to support children and families; Delivers interventions as needed and appropriate. Although the primary focus of this position is to provide services to the youth and families enrolled in the 1115 waiver Juvenile Diversion Program, this position may also assist with service delivery in other areas of the department. All positions within Behavioral Health Services are required to support the efficient functioning of the department. Participates in activities and duties related to emergency management during a local state of disaster as directed by appropriate county managers.

 

REQUIREMENTS:

Bachelor’s degree in psychology, social work or related field; Four (4) years experience providing services to youth who have been diagnosed with a mental illness or equivalent experience; Trauma training.

Proven proficiency with both oral and written communication skills. Organizational skills and the ability to complete multiple tasks a must. Strong interpersonal skills and the ability to deal effectively with the public, other employees and elected officials. Flexible work schedule is required in order to respond to clinical needs and other emergency situations. Spanish speaking preferred; Master’s degree preferred.

 

SALARY RANGE: Public Health Grade 11, $2,078.40 - $2,444.80 biweekly based on qualifications
